Covid-19 Waiver
Assumption of the Risk and Waiver of Liability Relating to Coronavirus
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local government and federal and state health agencies recommend social distancing.
By signing this agreement I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed or infected by COVID-19 by attending this Pico Shul event and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed or infected by COVID-19 while attending Pico Shul services may result from the actions, omissions, or negligence of myself or others, including, but not limited to, Pico Shul, Beth El Synagogue, the organizers and hosts, employees, volunteers and program participants and their families.
I understand that I have the option of not coming to this Pico Shul event or any other program or prayer service affiliated with the Pico Shul, Beth El Synagogue, the organizers and hosts. I understand that increased age and underlying medical conditions( heart, lung, kidney disease, autoimmune disorders, obesity and high blood pressure) can increase my risk of contracting COVID-19. I understand that being in close proximity to people, especially those who are singing (which causes more projection of droplets) increases my risk contracting COVID-19.
I am at least eighteen (18) years of age and voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury due to myself (including, but not limited to personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, or any kind that I may experience or incur in connection with my attendance at this Pico Shul event. I hereby release, covenant not to Sue, discharge, and hold harmless Pico Shul, Inc. & Pico Shul, Beth El Synagogue, the organizers and hosts, Inc.,their employees, agents and representatives of and from the claims, including all liabilities, claims, actions, disasters, costs or expenses of any kind arising out of or related to thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Pico Shul, Inc. & Pico Shul, Beth El Synagogue, the organizers and hosts, Inc. and it’s employees, agents and representatives, whether a COVID-19 infection occurs before, during or after participation in its event.
In the event I am infected with COVID-19, I hold the Pico Shul, Beth El Synagogue, the organizers and hosts harmless from any claims of any kind. I hereby assume all risk of contagion and any risks caused by any other person, which I acknowledge the Pico Shul, Beth El Synagogue, the organizers and hosts cannot control beyond requesting their cooperation in the above rules.
Symptoms
I also confirm that: I, nor anyone in my household have had any of the following symptoms in the last 5 days: sore throat, cough, chills, body aches for unknown reasons, shortness of breath for unknown reasons, loss of smell, loss of taste, fever at or greater than 100 degrees Fahrenheit.
If I do develop any of these symptoms within 5 days of the event, I will let the organizers know.
